  • Wedding, Funeral & Baby Dedication forms available in office ONLY
  • OFF-SITE MINISTRY REQUEST FORM: An off-site ministry requisition form should be used for all ministry events ( no matter the department) that will not take place on Christian Worship Center property.
  • ON-SITE MINISTRY REQUEST FORM: An on-site ministry requisition form should be used for all ministry events (no matter the department) that will take place on Christian Worship Center property. This form may be used for fundraisers and non-fundraising events.
  • PERSONAL FACILITIES USE FORM: This form is for personal use only (ie. personal events)
